CONSENT AGREEMENT & LIABILITY WAIVER

Zozi Nail Bar and Spa

By booking with Zozi Nail Bar and Spa and ticking the consent box, I acknowledge and accept responsibility for receiving services, which may include nail services (manicure, pedicure, nail extensions, polish), waxing, threading, massage, and head spa treatments.

I confirm and agree to the following:

  1. Assumption of Risk
    I understand that beauty and spa services may involve certain risks, such as irritation, redness, swelling, or sensitivity. I accept these risks and release Zozi Nail Bar and Spa, its owners, and staff from liability related to these Spa, its owners, and staff from liability related to these services.
  2. No Refund / No Chargeback Policy
    All services are final, non-refundable, and non-transferable. Once services are provided, I will not request refunds, chargebacks, or compensation, even if I am not satisfied with the results.
  3. Aftercare Responsibility
    I understand that proper aftercare is my responsibility. Any damage caused by pulling, peeling, picking, misuse, or accidents is not the responsibility of the salon or technicians.Zozi Nail Bar and Spa is not obligated to redo, repair, or refund services in such cases.
  4. Medical Conditions & Disclosure
    I confirm that I have disclosed any relevant medical conditions, such as allergies or skin sensitivities, and accept responsibility for not disclosing any condition.
  5. Adverse Reactions
    I understand that reactions may occur even with proper service and product use. If this happens, I agree to return to the salon for professional removal or seek medical attention if needed.
  6. Nail Extensions & Enhancements
    I understand that extending nails beyond their natural length may affect nail strength and may cause lifting, peeling, or breakage. The salon is not responsible for these outcomes.
  7. Massage, Head Spa & Waxing Risks
    I understand these services may cause temporary redness or sensitivity. I release the salon from liability for normal side effects or unforeseen reactions.
  8. Minors
    I consent to services for myself and/or my children under 18 years old, and accept responsibility for any outcomes related to these services.
  9. Release of Liability
    I release Zozi Nail Bar and Spa, its owners, employees, and contractors from all claims or demands arising from services received. This waiver applies to all known and unknown risks.
  10. Acknowledgment
    By proceeding with my booking, I confirm that I have read and understood this agreement, am of legal age to consent, and accept responsibility for my decision to receive services.
